Location: Elmont, New York
Job Type: Full-Time
Schedule: Monday – Friday, 9:00 AM – 5:00 PM
Salary: $120,000 – $145,000 annually (based on experience)

About Us
We are currently seeking a Family Nurse Practitioner (FNP) to provide comprehensive, evidence-based primary care services to patients across the lifespan—from pediatrics to geriatrics. This role involves health promotion, disease prevention, diagnosis, and management of both acute and chronic conditions, while emphasizing patient education and continuity of care.
The ideal candidate is a proactive, empathetic provider with strong clinical skills, a collaborative spirit, and a passion for community-based medicine
Responsibilities
- Conduct physical exams, health assessments, and patient evaluations across all age groups
- Diagnose and manage acute and chronic health conditions
- Prescribe medications and develop individualized treatment plans
- Provide preventive care services including immunizations, screenings, and patient counseling
- Maintain accurate and timely documentation in the Electronic Medical Record (EMR) system
- Collaborate with physicians, nurses, behavioral health providers, and care coordinators
- Participate in quality improvement initiatives and continuing education
- Educate patients on health management, lifestyle changes, and treatment adherence
Requirements
- Master’s or Doctorate degree in Nursing from an accredited program
- Current New York State RN and NP license
- National board certification as a Family Nurse Practitioner (AANP or ANCC)
- Current DEA registration
- BLS/CPR certification
- Minimum 1 year of primary care experience (new graduates with clinical rotations in family practice will also be considered)
- Strong communication and interpersonal skills
